What to expect:
Due to the Coronavirus’ impact on our Bay Area community, the way we visit with you has changed. Please be sure to follow the directions below. After this, our office team will help you understand your insurance options and benefits.
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. In your consultation, the surgical team provides a high-quality, individual assessment that walks you through your individual needs. Almost all treatment plans will require an appointment for an initial consultation and a second appointment to receive treatment on another day.
A preoperative consultation and physical examination are mandatory for patients undergoing general anesthesia for surgery: if you are coming in for an emergency consultation, it is imperative that you avoid all food and liquids for at least 8 hours. This allows you the most possible options for treatment.
If your meeting is in person:
- Your appointment will take place in our office, and we are excited to serve you! An abundance of care has been taken to provide a safe and sterile environment.
- Please take the opportunity to fill all your necessary documents out online, or print and bring them with you.

- Financial Policy
- Patient Health History and Insurance Information-
- This includes a copy of a photo ID, a copy of your medical insurance card, and a list of medications and supplements.
- Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, and etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, and etc.).
- Patient Referral Form with any associated images-
- If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
- Covid Screening
- Please arrive in our parking lot prior to your scheduled appointment. When you park, please call to let us know you got here safely. IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit. There are no exceptions to this rule.
- We meet you downstairs to escort you, so please remain in your vehicle until we give you further instructions. We will be checking your temperature and asking you about your current health before you walk in our doors. Because of this, please have everyone in your vehicle wear a mask.
- After you pass our COVID-19 screening, we show you to the bathroom to wash your hands.
- Finally, we welcome you inside our office for your consultation. We will honor physical distancing with you at all times*, and have taken an abundance of care to keep you safe and comfortable.
If your meeting is virtual:
- Your appointment is completely contact-free. We have taken an abundance of care to provide you with the best virtual experience available. The structure of this appointment is very similar to an in-person consultation. Because you elected this style of consultation, your intraoral exam has been postponed until the day of your procedure.
- Because your consultation is online, it is imperative that you fill out your paperwork 2 business days before your appointment.

- First, we ask that you give yourself a moment to get comfortable. Please have your device ready 3 minutes prior to your appointment. We recommend a quiet and distraction-free room, a comfy chair, and a computer screen. This will ensure you get the best experience possible.
- Next, we will reach out to you via Zoom Telehealth. This is a HIPAA compliant and secure service specifically designed for confidentiality.
- First, our team will guide you through your x-ray images via screen-sharing. This allows us to talk about potential treatment paths. The intraoral exam will confirm this on the day of your procedure.
- Finally, You will be talking to our office team about your insurance benefits and options.
